Necessary Forms for 2012 League and Skills Program

The following forms are necessary for the League and Skills Program

Please bring them with you, filled out, on your first day of attendance:

1) 2012 LEAGUE and SKILLS Registration Form

2) 2012 USAV Medical (Release) Form

3) 2012 Release Form, City of Newton

If you plan to attend in Jan., please email Stephanie Lefever (Head Coach/Director)  to tell her.  

This program is designed primarily for High School players (if younger, email Stephanie Lefever)

There is a "Tryout Fee" of $40 for the 1st 2 sessions, Jan. 8 and Jan. 15.  

These are dates that players 'try us out', and do not require the full payment for the season.

If a player paid the tryout fee for the Travel Team Tryouts, and was not selected, the Tryout Fee for the

League and Skills Program is waived!
