   You must submit them AT THE TRYOUTS!

1) 2007-2008 Travel Team Registration Form (bring to Tryouts)

2) 2007-2008 USAV Medical and Release Form (bring to Tryouts)

3) 2007-2008 City of Newton Release Form (bring to tryouts)

4) 2007-2008 Player Code of Conduct (final team members)

5) 2007-2008 USAV Membership Form

 NOTE: this last form, USAV Membership, has its own method (click here)  use Form 5) here even if you are able to join USAV online.  Forms 1-5 are still required at the tryouts! IMPORTANT! How to submit Required USAV online Registration click here
